The Coast Guard is serious about wanting to keep track of all Team Coast Guard members.

That includes the Auxiliary, so make sure your calling tree information is up to date. 

Please submit a 7028 (Change of Member Information)** to the FSO-IS when your contact information changes.  

In the event of hurricanes or other disaster there are now two numbers available to let the Coast Guard know your status.

If you plan to evacuate, and know where you are going, call and provide that information. 

If you don't know your destination before hand, call when you reach your point of refuge.

The 7028 form does not provide the same built-in email function as some other CGAux forms such as the 7029 or 7030. 
Please fill out the form then print it and hand a hard copy to the FSO-IS.
